The Dragonstone of Bleakfalls Barrow to Farengar. He only had to send me on that quest once. I have it every time thereafter. That's approximately 300 times.

Witcher 3 and Fallout 4 do have some of those checks. The quests behave slightly different if you have that item already. While other games just run their standard routine (give quest, flag it, turn in).

in KCD2 the huntsmaster asks you to bring him sheep guts for a quest. if you tell him you already have some he hits you with ' jesus, are you some kind of freak?!'
I dislike games where the quest item appears only after activation, forcing a return to a previously explored area.
And they desperately needed 3 pieces of some herbal to save their dying child, which you had 387 pieces in your inventory already because it grows absolutely everywhere.
